Immunization Records

The state law and our Board of Education policy requires that all entering school students meet the requirements for immunization prior to their entering school.  A student will not be allowed to attend class without written proof of immunization.

                   Polio:  4 doses at any age, but…3 doses meet requirement for ages 4-6; if at least one was given on or after the 4^th birthday; 3 doses meet requirement for ages 7-17 years if at least one was given on or after the 2^nd birthday.

                   DPT:  5 doses at any age, but…4 doses meet requirements for ages 4-6 years if at least one was on or after the 4th birthday. 4 doses at any age, but…3 doses meet requirement for ages 7-17 years if at least one was on or after the 2^nd birthday. If last dose was given before the 2^nd birthday, one more (Td) dose is required.

For 7^th graders – a booster is recommended if more than 5 years have passed since last DTP, DtaP, DT, or Td dose.

                   MMR: Kindergarten: 2 doses both on or after 1^st birthday. 7^th grade: 2 doses both on or after 1^st birthday. Grades 1-6 and 8-12: 1 dose must be on or after 1^st birthday.

                   HepB: Kindergarten: 3 doses; 7^th grade 3 doses. All 7^th graders New-to-district must have HepB or have begun the series.

Varicella (Chicken Pox):  Kindergarten: 1 dose; Out-of-state entrants (grades 1-12) 1 dose for children under 13 years; 2 doses are needed if immunized on or after 13^th birthday.

ALL Seventh graders entering or in the district must have all three doses of  HepB.
